改良Ashworth量表概况
改良Ashworth量表(Modified Ashworth Scale,MAS)被认为是测量神经疾病患者肌肉痉挛的主要临床工具,被许多人视为测量痉挛的金标准。然而,一些文献质疑其测量痉挛的能力,主张将其作为测量肌张力异常或被动运动阻力的等级量表,因为目前临床上没有直接测量痉挛的方法(Gregson, Leathley, Moore, Sharma, Smith & Watkins, 1999; Pandyan, Johnson, Price, Curless, Barnes & Rodgers, 1999)。该量表为评估工具而非筛查工具,共6个等级(0-4分,含1+),证据审核时间截至2011年7月13日之前。
改良Ashworth量表的目的
MAS用于临床测量肌肉痉挛,或更准确地说,是测量肌张力异常(被动运动阻力)的等级量表。原版Ashworth量表由Bryan Ashworth于20世纪60年代初开发,用于评估抗痉挛药物对多发性硬化患者的疗效,为5级量表(评分0、1、2、3、4)(Ashworth, 1964)。1987年,Bohannon和Smith增加了「1+」等级,并对各等级定义稍作修改,以提高量表的敏感性并便于评分,即改良Ashworth量表(Bohannon & Smith, 1987)。
改良Ashworth量表的测试项目与分量表
虽然无标准化使用指南,MAS可应用于上肢或下肢的肌肉。评估者应将患者肢体从最大屈曲位伸展至最大伸展位,直至感受到第一次软性阻力。将患者肢体通过全关节活动范围(ROM)应在1秒内完成(默数one thousand and one)(Bohannon and Smith, 1987)。该量表无分量表。
上肢测试(Mehlroz, Wagner, Meibner, Grundmann & Zange, 2005建议):患者仰卧,上肢与躯干平行,肘伸直,腕中立位,双下肢相互平行。例外:肩伸肌——手臂从伸展位移至90度屈曲;肩内旋肌——手臂从中立位移至最大外旋位。
下肢测试(Blackburn, van Vliet, & Mockett, 2002建议):患者侧卧。比目鱼肌:髋、膝屈曲45度,踝关节由最大跖屈移至最大背屈;腓肠肌:髋屈曲45度,膝最大伸展,踝关节由最大跖屈移至最大背屈;股四头肌:膝、髋最大伸展位,膝关节由最大伸展移至最大屈曲。
测试全程应嘱患者保持平静放松;如需重复测试,应在每天同一时间进行,以减少因药物相互作用导致痉挛水平变化的影响(Bohannon and Smith, 1987)。
改良Ashworth量表的施测规则
评估者将患者肢体从最大屈曲位伸展至最大伸展位,直至感受到第一次软性阻力,1秒内完成全关节活动范围。施测时间未报告,随测试肌肉数量不同而变化。测试全程应嘱患者保持平静放松;如需重复测试,应在每天同一时间进行。
改良Ashworth量表的评分方法
MAS为6级量表,评分范围0-4(含1+)。低分代表肌张力正常,高分代表痉挛或被动运动阻力增加。各等级定义如下:
- 0分:肌张力无增加。
- 1分:肌张力轻度增加,表现为卡顿后释放,或受累部位屈/伸时在关节活动范围末端出现最小阻力。
- 1+分:肌张力轻度增加,出现卡顿,随后在关节活动范围其余范围(不足一半)内出现最小阻力。
- 2分:肌张力较明显增加,累及大部分关节活动范围,但受累部位仍易于被动活动。
- 3分:肌张力显著增加,被动运动困难。
- 4分:受累部位屈/伸位僵直。

改良Ashworth量表的心理测量学特性
文献检索共识别出20项针对脑卒中患者MAS心理测量学特性的相关研究。MAS在信度和效度方面的研究结果存在冲突。MAS的信度似乎因肌肉而异:总体而言,肘关节和腕关节的评估结果优于膝关节和踝跖屈肌群的评估。
评定者内信度(4项研究,均报告合格至优秀):Gregson, Leathley, Moore, Sharma, Smith, & Watkins(1999)在32例急性脑卒中患者(中位年龄74岁)中测量肘部肌张力,同一评定者在1天间隔内、每天同一时间评估,加权kappa=0.83(优秀)。Gregson, Leathley, Moore, Smith, Sharma, & Watkins(2000)在35例急性脑卒中患者(中位年龄75岁)中测量肘、腕、膝、踝的屈肌与伸肌肌张力:加权kappa肘=0.83(优秀)、腕=0.88(优秀)、膝=0.94(优秀)、踝=0.64(合格)。Blackburn, van Vliet, & Mockett(2002)在20例急性脑卒中+16例慢性脑卒中患者中测量腓肠肌、比目鱼肌、股四头肌(1周间隔):Kendall's tau-b总体=0.56(合格),腓肠肌=0.44,比目鱼肌=0.58,股四头肌=0.66;MAS低分一致性更高,0分时评定者内一致率为60%,2分时仅12%。Mehrholz, Wagner, Meibner, Grundmann, & Zange(2005)在30例重症脑损伤患者(脑卒中、颅脑外伤或脑缺氧)中测量肩、肘、腕、髋、膝、踝肌张力(1天间隔、同一时间和体位):各肌肉kappa均为合格(肩屈肌0.55、肩外旋肌0.47、肘屈肌0.47、肘伸肌0.53、腕屈肌0.58、腕伸肌0.51、髋屈肌0.53、髋伸肌0.49、膝屈肌0.52、膝伸肌0.55、踝伸肌(膝屈曲位)0.62、踝伸肌(膝完全伸展位)0.47);与改良Tardieu量表(MTS)比较,除肩部伸肌和内旋肌外(P>0.05),MTS的评定者内信度在所有肌肉上均显著更高(P<0.05)。
评定者间信度(8项研究,报告为差至优秀):Bohannon and Smith(1987)在30例颅内病变患者(多发性硬化n=1、闭合性颅脑损伤n=5、脑卒中n=24)中测量肘屈肌肌张力,Kendall's tau-b=0.84(优秀)。Bodin and Morris(1991)在18例脑卒中患者中测量腕部肌张力(3种条件下),Kendall's tau-b=0.85(优秀),kappa=0.74(合格)。Sloan, Sinclair, Thompson, Taylor, & Pentland(1992)在34例脑卒中后偏瘫患者中测量膝屈肌、肘屈肌及伸肌(4名评定者),Spearman's rho肘=0.56-0.90(合格至优秀),膝=0.26-0.62(差至优秀)。Gregson et al.(1999)在32例急性脑卒中患者中测量肘部(2名评定者),加权kappa=0.84(优秀)。Gregson et al.(2000)在35例急性脑卒中患者中测量肘、腕、膝、踝屈伸肌(2名评定者),加权kappa肘=0.96(优秀)、腕=0.89(优秀)、膝=0.79(优秀)、踝=0.51(合格)。Blackburn et al.(2002)在20例急性+16例慢性脑卒中患者中测量腓肠肌、比目鱼肌、股四头肌(2名评定者),Kendall's tau-b总体=0.06(差),腓肠肌=0.15,比目鱼肌=0.19,股四头肌=0.28(均差);评定者间一致主要发生在0分(一致率40.8%),高分(如2分)一致率为0%。Ansari, Naghdi, Moammeri, & Jalaie(2006)在15例脑卒中患者中测量肘屈肌(2名评定者),kappa=0.21(差)。Mehrholz et al.(2005)在30例重症脑损伤患者中测量肩、肘、腕、髋、膝、踝(4名评定者):kappa肘伸展=0.42(合格),其余肌肉均差(肩屈肌0.29、肩外旋肌0.16、肘屈肌0.33、腕屈肌0.34、腕伸肌0.30、髋屈肌0.31、髋伸肌0.24、膝屈肌0.28、膝伸肌0.35、踝伸肌(膝屈曲位)0.20、踝伸肌(膝完全伸展位)0.14);与MTS比较,除腕伸肌外(P>0.05),MTS的评定者间信度在所有肌肉上均显著更高(P<0.05)。
内容效度(1项研究,提示需谨慎):Pandyan, Johnson, Price, Curless, Barnes, & Rodgers(1999)通过文献综述收集MAS的理论依据,指出MAS的隐含假设为:被动运动阻力的变化源于痉挛的变化;重复测量时肌肉的牵张机械感受器以相似速度延伸;重复测量时各关节的活动范围不变。作者指出,宣称MAS是痉挛测量工具时需谨慎,因为证据表明被动运动阻力并非痉挛的专属指标——阻力还会随主动肌与拮抗肌α运动神经元的兴奋水平、软组织和关节的粘弹性特性而变化。
同时效度(1项研究,与表面肌电图相关性差):Cooper, Musa, van Deursen, & Wiles(2005)在31例脑卒中患者+20名健康人中以表面肌电图为痉挛金标准与MAS比较,Spearman's rho=0.21(相关性差)。预测效度:无研究检验。
聚合/区分效度(5项研究):Katz, Rovai, Brait, & Rymer(1992)在10例脑卒中患者中与Fugl-Meyer评估及痉挛客观测量(肌电图、力矩、钟摆试验、H/M比值)比较(Pearson相关):Fugl-Meyer评估r=-0.94(优秀),肌电图r=-0.79(优秀),钟摆试验r=-0.67(优秀),与力矩、H/M比值无显著相关(数值未报告)。Lin and Sabbahi(1999)在10例慢性脑卒中患者中与牵张反射亢进指标(肌电图、力矩反应、牵张反射的速度敏感性)及运动表现指标(FMA、Box and Block试验、主动关节活动范围、握力)比较,分两天测量(Spearman's rho):FMA第1天-0.83、第2天-0.76(优秀);Box and Block试验第1天-0.83、第2天-0.76(优秀);主动关节活动范围第1天-0.74、第2天-0.62(优秀);握力第1天-0.86、第2天-0.85(优秀);静息肌电图第1天0.77、第2天0.67(优秀),主动肌肉肌电图第1天0.77、第2天0.74(优秀),静息肌力矩第1天0.80(优秀);速度敏感性第1天0.52、第2天0.57(合格);静息肌力矩第2天-0.25、主动肌力矩第1天0.26、第2天0.21(均差)。Pandyan, Price, Rodgers, Barnes & Johnson(2001)在16例急性脑卒中患者中与肘部被动运动阻力的生物力学测量比较,kappa=0.36(差)。Pandyan, Price, Barnes, & Johnson(2003)在63例脑卒中患者中与肘部被动运动阻力生物力学测量比较,Spearman's rho=0.51(合格)。Pizzi, Carlucci, Falsini, Verdesca & Grippo(2005)在65例脑卒中患者中与痉挛的神经生理学评估(H反射与M反应)、肘/腕被动关节活动范围及疼痛比较:与腕部神经生理学评估rho=0.40(合格);MAS高分(大于3)与被动关节活动范围下降相关(F=6.8);疼痛与MAS评分(肘或腕)之间无相关。
已知组效度(2项研究,均显示无法区分):Bakheit, Maynard, Curnow, Hudson, & Kodapola(2003)在24例脑卒中患者中采用student t检验,结果显示MAS无法区分H反射潜伏期及H/M比值较高与较低的个体。Kumar, Pandyan, & Sharma(2006)在111例脑卒中患者中采用方差分析(ANOVA),结果显示MAS无法区分4种僵硬度水平(无、轻、中、重度僵硬)的个体。
反应性:无研究检验过MAS的反应性(即检测患者临床重要变化的能力)。内部一致性:无研究检验。重测信度:无研究检验。
改良Ashworth量表的版本
Ashworth量表(Ashworth Scale,1964年发表):原始的被动运动阻力测量工具,5级量表(0-4),敏感性低于MAS(Brashear, Zafonte, Corcoran, Galvez-Jimenez, Gracies, Gordon et al., 2002)。
改良Ashworth量表(Modified Ashworth Scale,MAS,1987年发表):Bohannon和Smith增加「1+」等级并稍作定义修改,被视为测量痉挛的金标准。
再改良Ashworth量表(Modified Modified Ashworth Scale,MMAS,2006年发表):旨在提高MAS的信度,为5级量表,删去了「1+」等级并略微重新定义了「2」级(Ansari, Naghdi, Younesian, & Shayeghan, 2008)。各等级定义:0分=肌张力无增加;1分=肌张力轻度增加,表现为卡顿后释放,或受累部位屈/伸时在关节活动范围末端出现最小阻力;2分=肌张力明显增加,在关节活动范围中段出现卡顿,但受累部位仍易于活动;3分=肌张力显著增加,被动运动困难;4分=受累部位僵直于屈/伸位。
