卒中影响量表概况
卒中影响量表(Stroke Impact Scale,SIS)是一个脑卒中特异性、自评式健康状况量表,用于评估多维度的脑卒中结局,包括力量、手功能、日常生活活动/工具性日常生活活动(ADL/IADL)、移动能力、交流、情绪、记忆与思维以及参与,可用于临床和研究场景。SIS属于评估工具,由堪萨斯大学医学中心Landon老年中心开发,最早以版本2.0形式由Duncan、Wallace、Lai、Johnson、Embretson和Laster于1999年发表。版本2.0包含64个条目、8个维度;基于Rasch分析结果,从2.0版删除5个条目,形成现行版本3.0。施测时间约15-20分钟,无需正式培训。本页证据审查截至2018年6月29日之前。

卒中影响量表的测试项目与分量表
SIS版本3.0共59个条目,评估8个维度,各维度条目数如下:

- 手功能(Hand function)——5个条目
- 日常生活活动/工具性日常生活活动(ADL/IADL)——10个条目
- 移动能力(Mobility)——9个条目

卒中影响量表的施测规则
施测前必须照原文朗读目的声明,并告知患者信息基于其本人观点。目的声明内容为:本问卷的目的是评估脑卒中对您的健康和生活造成的影响。我们想从您的角度了解脑卒中如何影响了您。我们将询问您脑卒中造成的损伤和残疾,以及脑卒中如何影响您的生活质量。最后,我们将请您评估您认为自己从脑卒中中恢复了多少。
应随量表提供大字版答题纸,使受访者可同时看到和听到每个问题的选项。受访者可用数字或对应文字作答(如「5」或「完全不困难」);若用数字作答,访谈者须复述对应文字以核实答案。访谈者应展示相应题组的答题纸,且每题后必须朗读全部五个选项。
问题按维度分组,每组前有概括性描述(如「这些是关于您的脑卒中所导致的躯体问题」);每组有特定时间参照语句(如「在过去一周,您如何评价您……的力量」),该语句须在每个问题前重复。量表中的时间周期在1周、2周、4周之间变化,须向受访者强调所评估时间期的变化。
卒中影响量表的评分方法
SIS为患者自评问卷,每个条目采用5点Likert量表:1=无法完成该项;5=完全没有困难。反向计分注意:情绪维度的3个条目(3f、3h、3i)在计算该维度得分前须反向计分(即1对应5、2对应4、3等于3、4对应2、5对应1);SIS计分数据库已自动处理此反向,手工计分时使用公式:条目得分=6-个人评分。
恢复域为单一条目,采用0-100的视觉模拟量表:0=无恢复,100=完全恢复。各维度得分范围为0-100分,计算公式为:维度得分=[(平均条目得分-1)/(5-1)]×100。得分解释采用与SF-36(Ware与Sherbourne,1992)等效的算法生成各维度总分。计分数据库可在堪萨斯大学医学中心网站下载。

卒中影响量表适合哪些患者群体?
仅可施测于脑卒中患者。SIS 3.0和SIS-16可通过电话、邮寄、代理、代理邮寄方式完成(Duncan等,2002a;Duncan等,2002b;Kwon等,2006);研究显示代理作答在躯体维度可能存在代理偏倚(Mulder与Nijland,2016),建议权衡可能的应答者偏倚、代理使用的固有困难与邮寄调查的经济优势。慎用于:(1)SIS 2.0在轻度损伤个体中应慎用——交流、记忆和情绪维度条目偏容易,仅能捕捉最重度损伤者的受限(Duncan等,2003);(2)受访者必须能执行三步指令(Sullivan,2014);(3)对脑卒中后有注意力、专注力或疲劳困难的个体,施测时间较长是限制因素(MacIsaac等,2016)。
卒中影响量表的心理测量学特性
通过文献检索识别SIS心理测量学特性的相关发表文献,共纳入17项研究,仅针对原始英文版SIS 2.0、SIS 3.0或SIS-16。
地板/天花板效应:Duncan等(1999)报告SIS 2.0在中度脑卒中组手功能域存在潜在地板效应(40.2%);交流域在轻度(35.4%)和中度(25.7%)脑卒中组可能出现天花板效应;SIS天花板效应最高的是交流域(35%),相比之下Barthel指数的天花板率高达64.6%。Duncan等(2003b)的Rasch分析证实了上述两个效应,另报告记忆和情绪域存在天花板效应。Lai等(2003)对278例脑卒中后3个月患者检查SIS-16和SIS社会参与域:SIS-16地板/天花板效应分别为0%和4%;社会参与域为1%和5%。Richardson等(2016)对164例亚急性脑卒中患者于三个时间点测量:较差的天花板效应(大于20%)见于手功能域基线/6月/12月(25.0%、36.4%、37.8%)、记忆与思维域6月/12月(22.2%、21.6%)、交流域6月/12月(30.6%、27%)、移动域6个月(20.4%)、ADL/IADL域12个月(21.6%);任何时间点均无显著地板效应。
信度-内部一致性:Duncan等(1999)报告SIS 2.0的Cronbach系数alpha=0.83-0.90,8个维度均为优秀。Duncan等(2003b)报告SIS 2.0的Rasch分析条目分离信度0.93-1.00,8个维度中有5个(加复合躯体维度)分离指数超过2.00(相当于alpha大于等于0.80,优秀);情绪和交流域因天花板效应仅为充分水平,手功能域因地板效应仅为充分水平。Edwards与O'Connell(2003)对74例脑卒中患者报告内部一致性优秀(alpha=0.87(参与)至alpha=0.95(手功能))。Lai等(2003)报告SIS-16和社会参与域条目难度分布良好,较易条目可测量重度脑卒中患者较低水平的躯体功能。Jenkinson等(2013)报告SIS 3.0(n=73)所有维度内部一致性优秀(alpha=0.86-0.96),高阶因子分析显示一个特征值大于1的因子解释68.76%方差;SF-SIS(n=151)内部一致性高(alpha=0.89),单因子解释57.25%方差。Richardson等(2016)报告所有维度在所有时间点内部一致性均优秀(alpha=0.81-0.97),复合躯体功能总分在所有时间点优秀(alpha=0.95-0.97)。MacIsaac等(2016)报告急性脑卒中5549例、脑卒中康复332例两个数据集总体内部一致性优秀(alpha=0.98、0.93),各维度优秀,例外为情绪域(alpha=0.60、0.63)和力量域(仅康复数据集alpha=0.77)。
信度-重测信度:Duncan等(1999)对25例患者于脑卒中后3或6个月施测、一周后再次施测,ICC为充分至优秀(ICC=0.7-0.92),例外为情绪域相关性差(ICC=0.57)。
效度-内容效度:SIS的开发基于堪萨斯大学医学中心Landon老年中心的一项研究(Duncan、Wallace、Studenski、Lai与Johnson,2001),采用患者个别访谈及患者、照护者和医护专业人员焦点小组访谈的反馈。参与者包括30例轻中度脑卒中患者、23名照护者、9名脑卒中专家。共识小组审核候选条目、确定维度、制定条目量表,并决定施测与计分机制。

效度-效标效度(预测效度):SIS 2.0的躯体功能、情绪和参与维度是患者自评恢复的显著预测因子;SIS得分对日均步数预测较差。SIS 3.0治疗前得分与3周上肢康复后结局比较——手功能和ADL/IADL维度与FIM、FMA、MAL-AOU、MAL-QOM、FAI、NEADL充分至优秀相关,其他维度为差至充分。SIS-16入院得分与实际住院时长优秀相关、与预测住院时长充分相关;与出院去向(回家/康复)显著相关;MAL-QOM与SIS的早期结局联合使用时,预测脑卒中患者最终生活质量的准确性高。
效度-结构效度(聚合/区分效度):SIS 2.0各维度与对应WHOQOL-BREF分量表及Zung自评抑郁量表充分至优秀相关;交流维度与WHOQOL-BREF和Zung量表相关差;躯体维度与WHOQOL-BREF环境相关差。SIS 3.0与SF-SIS、EQ-5D、改良Rankin量表、Barthel指数、NIHSS优秀相关;与EQ-VAS中等至优秀;与SIS-VAS中等。SIS 3.0电话调查版与FIM和SF-36V充分至优秀。SIS-16与WHOQOL-BREF躯体域充分至优秀,与WHOQOL-BREF社会关系域差。SF-SIS与EQ-5D、改良Rankin量表、Barthel指数、NIHSS优秀相关;与EQ-VAS中等至优秀;与SIS-VAS中等。
效度-已知组效度:SIS 2.0多数维度可区分不同脑卒中严重程度患者;SIS 3.0躯体和ADL/IADL维度对不同严重程度有分数区分度和分布;SIS-16可区分不同脑卒中严重程度患者。
反应性:SIS 2.0一项研究报告脑卒中后1个月与3个月、1个月与6个月之间患者恢复出现预期方向的显著变化,但对变化的敏感性受脑卒中严重程度和评估时间点影响。SIS 3.0:一项研究确定了力量、ADL/IADL、移动能力、手功能四个分量表的临床重要差异变化值:最小可检测变化MDC分别为24.0、17.3、15.1、25.9,最小临床重要差异分别为9.2、5.9、4.5、17.8;一项研究报告手功能、脑卒中恢复和SIS总分为中等反应性,其余维度反应性小;一项研究发现参与和恢复是脑卒中后第一年内反应性最强的维度,力量和手功能也显示高的临床有意义的正/负变化。SIS-16:一项研究报告变化分数23.1表示入院至出院有统计学显著改善,对变化的敏感性大。
可接受性:SIS 3.0和SIS-16均有代理版本;以患者为中心的量表开发过程可增强其对患者的相关性,多水平评估可降低患者负担;施测时间较长被指出是限制。可行性:患者自评量表,施测15-20分钟;可当面、代理、邮寄或电话施测;无需正式培训。
卒中影响量表的版本
SIS由堪萨斯大学医学中心Landon老年中心开发。最早以版本2.0形式由Duncan等(1999)发表(64个条目、8个维度);基于Rasch分析结果删除5个条目,形成现行版本3.0(59个条目)。替代版本包括:
SIS-16(Duncan等,2003a):为解决脑卒中功能结局测量中躯体功能差异敏感度不足的问题而开发。SIS 2.0因子分析显示4个躯体维度(力量、手功能、ADL/IADL、移动能力)高度相关,可合并为单一躯体维度总分(Duncan等,1999;Mulder与Nijland,2016)。SIS-16由SIS 2.0中16个条目组成:ADL/IADL 7条、移动能力8条、手功能1条,其余维度应保持独立。
SF-SIS(Jenkinson等,2013):简版SIS,含8个条目(每个维度选取与该维度总分相关性最高的1条)。选取方法有三种:初步试点研究、验证分析、焦点小组;最终条目为被2种及以上方法选中者。已在急性与康复脑卒中患者焦点小组及多学科脑卒中医护团队中评估表面效度和可接受性,并已评估内容效度、聚合效度与区分效度(MacIsaac等,2016)。
语言版本:原始开发语言为英语。已进行文化适应、翻译及心理测量学检验的语言包括:巴西葡萄牙语(Carod-Artal等,2008)、法语(Cael等,2015)、德语(Geyh、Cieza与Stucki,2009)、意大利语(Vellone等,2010;Vellone等,2015)、日语(Ochi等,2017)、韩语(Choi等,2017;Lee与Song,2015)、尼日利亚豪萨语(Hamza等,2012;Hamza等,2014)、葡萄牙语(Goncalves等,2012;Brandao等,2018)、乌干达语(Kamwesiga等,2016)、英国英语(Jenkinson等,2013)。MAPI研究院已将SIS和/或SIS-16翻译为众多语言(包括南非荷兰语、阿拉伯语、保加利亚语、粤语、捷克语、丹麦语、荷兰语、波斯语、芬兰语、法语、德语、希腊语、希伯来语、匈牙利语、冰岛语、意大利语、日语、韩语、马来语、普通话、挪威语、葡萄牙语、俄语、斯洛伐克语、西班牙语、瑞典语、他加禄语、泰语、土耳其语等),这些翻译可能未经效度验证。
