蒙哥马利-阿斯伯格抑郁评定量表概况


蒙哥马利-阿斯伯格抑郁评定量表(Montgomery Asberg Depression Rating Scale,MADRS)供临床医生使用,用于评估已确诊抑郁症患者的抑郁严重程度。该量表专为对抗抑郁药治疗引起的变化敏感而设计,由Stuart Montgomery和Marie Asberg于1979年开发,基于Asberg的综合精神病理评定量表(Comprehensive Psychopathological Rating Scale,CPRS)发展而来,该原量表旨在评估精神科治疗效果。MADRS为评估工具而非筛查工具,共10个条目,总分0-60分,访谈施测需20-60分钟,证据审核时间截至2009年9月21日之前。

MADRS用于评估已确诊抑郁疾病患者的抑郁严重程度,并对治疗引起的变化敏感。最初发表的MADRS不含建议提问;2008年Williams和Kobak开发了SIGMA(Structured Interview Guide for the MADRS,MADRS结构化访谈指南),提供须逐字照读的结构化问题以保证施测标准化,并附澄清症状的追问问题。使用SIGMA时,评定者间信度(组内相关系数)极佳(r=0.93)。

每个条目严重程度评分为0-6分,分数越高症状越严重,总分范围为0-60分。Snaith, Harrop, Newby, and Teale(1986)提出的界值:0-6分无症状,7-19分轻度抑郁,20-34分中度抑郁,35-60分重度抑郁。

蒙哥马利-阿斯伯格抑郁评定量表适合哪些患者群体?


可用于:脑卒中患者;失语症患者——但一项研究发现MADRS比DSM-IV更难完成,尤其是完全性、混合性非流利型及Wernicke失语患者,且需要代理人代答时MADRS效度下降(Laska, Martensson, Kahan, von Arbin, & Murray, 2007)。


不应用于:MADRS仅适用于已确诊抑郁疾病的患者。


慎用:认知障碍患者可使用但须谨慎,某些条目措辞复杂(尤其是注意集中困难、无法感受、倦怠、内心紧张),可能使分数偏向更高的抑郁评分(Sadavoy, Smith, Conn, & Richards, 2004)。

文献检索共识别出4项针对脑卒中人群的相关研究。


地板/天花板效应:脑卒中人群无相关研究。Fantino and Moore(2009)在278例重性抑郁障碍门诊患者中使用MADRS-S 9条目自评版:天花板效应各条目均良好(总分0.4%),地板效应从差(食欲条目21.6%)到极佳(总分0.0%)不等。


信度——内部一致性:Agrell and Dehlin(1989)在40例老年脑卒中患者(其中17例经临床检查符合抑郁)中比较6种抑郁评定量表,MADRS所有条目均显著相关,8/10条目高度相关(文中未提供系数),Cronbach's alpha=0.89(极佳)。注:该文献将MADRS称为Comprehensive Psychopathological Rating Scale-Depression(CPRS-D),此为MADRS原名(Asberg, Montgomery, Perris, Schalling, & Sedvall, 1978)。重测信度:脑卒中人群无相关研究;Fantino and Moore(2009)在278例重性抑郁障碍门诊患者(自评版)中报告组内相关系数r=0.78(极佳)。评定者内信度:脑卒中人群无相关研究。评定者间信度:脑卒中人群无相关研究;Montgomery and Asberg(1979)在64例原发性抑郁疾病患者中的比较(两名英国评定者、两名瑞典评定者、一名英国与一名瑞典评定者、受训精神科医生与全科医生、受训精神科医生与精神科护士)显示相关系数r=0.89至r=0.97(极佳);SIGMA版本评定者间信度r=0.93(极佳)。


效度——内容效度:MADRS覆盖抑郁核心症状,唯一例外为精神运动性迟滞,因其相对少见而被排除于初始条目筛选(Montgomery & Asberg, 1979)。同时效度:Agrell and Dehlin(1989)在40例老年脑卒中患者中(Pearson积差相关):与老年抑郁量表(GDS)r=0.86(极佳),与Zung量表r=0.82(极佳),与CES-D r=0.83(极佳),与Hamilton量表(HRS)r=0.87(极佳),与Cornell量表相关性适中(该量表与各量表相关性均不高)。Tamaklo, Schubert, Mentari, and Lee(1992)在22例脑卒中患者中与Beck抑郁问卷(BDI)相关r=0.65(极佳)。


结构效度:Farner, Wagle, Flekkoy, Wyller, Fure, Stensrod, et al.(2009)对163例脑卒中患者的因子分析识别出3个独立因子:快感缺失(与认知损害相关)、悲伤(与脑卒中导致的神经功能损害相关)、激越(与不直接源于脑卒中的躯体因素相关)。


敏感度/特异度:Sagen, Vik, Moum, Morland, Finset, and Dammen(2009)在104例脑卒中后4个月患者中以DSM-IV抑郁诊断为金标准:界值大于6分时敏感度0.90、特异度0.66;界值大于12分时敏感度0.70、特异度0.86;所有低于9的界值敏感度大于0.80、特异度大于0.60;界值大于8分时总体一致性最高(0.74),kappa=0.40,阳性预测值=0.41;AUC=0.91(极佳)。


反应性:脑卒中人群中尚未检验。可接受性:通常访谈施测,可自评;认知障碍患者慎用(分数可偏高);可用于失语症患者。可行性:SIGMA使施测简便;无需特殊培训或设备。

MADRS(1979年,源自CPRS):原版访谈量表。MADRS-S自评版:由Svanborg和Asberg于1994年开发,含9个条目,基于过去3天的感受,条目为:情绪、不安感、睡眠、食欲、专注能力、主动性、情感投入、悲观、生活热情。SIGMA结构化访谈指南(2008年):提供须逐字照读的结构化问题以保证施测标准化。
